Sen.-elect Brown may be health care reform’s unwitting savior

Do you have reliable, affordable, high-quality health care, guaranteed for life?

Do all of your children? Parents? Grandchildren? Friends and neighbors?

If so, congratulations.

If not, like many of us here in New Hampshire, you probably lie awake late into the night sometimes worrying about what kind of havoc an illness or accident might wreak on your family’s finances.

Next time you can’t sleep, do what I do and try running these numbers through your head:

  • $936,267,403: The amount donated to Congress by the health care industry.
  • $20,915,072: The amount donated to President Barack Obama by the health care industry.
  • 15,385: The average number of Americans who lost their health insurance each day in 2009.
  • 3,330: The number of health care industry lobbyists who played a role in writing the current health care “reform” bill.
  • 428 percent: The increase in profits at 10 of the nation’s largest health insurance companies between 2000 and 2007.
  • 72 percent: The percentage of Americans who support universal health care coverage, according to a CBS News/NY Times poll.
  • 51.9 percent: The percentage of votes won by Massachusetts’ newest senator, Scott Brown, as he took the late Ted Kennedy’s seat and eliminated the Democrat’s filibuster-proof margin.

Ironically, it’s this last number that stands the best chance of saving us from all the others. But only if we stand up right now and demand our rights as Americans.

Political pundits are struggling to make sense of the Massachusetts vote, which ended a 50-year winning streak for the Democrats and which by all accounts will jeopardize the passage of Obama’s health care reform bill.

Good.

Because I want health care reform. My friends and colleagues want it. My kids really want it. And 72 percent of Americans want it, too.

But the current bill is utterly unworthy of the word “reform.” Every life-saving ounce of real reform was either suppressed in advance, stripped from the legislation, or bargained away in the usual political haggling on the Hill.

Washington might think it can dress up this considerably flawed, deeply unpopular piece of compromise as “reform,” but the American people are far too smart for that. For starters, this bill:

n Relies on a rapacious industry to ensure that our quality of and access to health care are on an equal footing with their profit margins.

n Forces us to purchase health insurance without any safeguards against insurance companies dropping unprofitable patients or raising premium costs, deductibles and co-pays.

n And requires us to pay for this on “lay-away” with health care benefits kicking in only well after we start paying the premiums.

Congress and the administration are hurriedly considering a number of ways to get this bill passed into law before Brown – who may go down in history as health care reform’s unwitting savior – is sworn into office.

Their window of opportunity is a few weeks at most. Which means it is time to do or die – time to kill this miserable excuse for health care reform and create something that will truly improve this nation’s health and well being.

“Of all of the forms of inequality,” said the Rev. Dr. Martin Luther King Jr., “injustice in health care is the most shocking and inhumane.”

It is the right of every American – rich or poor, young or old, rural or urban – to have access to affordable, quality health care. Seriously, what’s the point of being the world’s most powerful and wealthy nation if we won’t take decent care of our people?

Universal single-payer health care will cut overhead and administrative costs, premiums, waste, duplication, lawsuits and the growing expense of insurance company liaison staff.

Most importantly, universal health care will once and for all cut off the control of our health care by the profit-motivated health care insur-ance industry and put it in the hands of a government of, for and by the people.

The current bills that have passed in Congress do none of the above. And no matter how they are mish-mashed together, the outcome will never come near to producing what a universal single-payer health care program guarantees.

We need to kill the congressional health care conciliation bill and demand that our elected representatives go back to the drawing board and produce what we sent them there to do, not what the 3,000-plus lobbyists bought with their $9 million-plus dollars.

Thanks, Sen.-elect Brown, for giving us another chance to get it right!

Rising to President Obama’s challenge in his State of the Union address that others come up with a better approach to health care reform than his own, two Maryland physicians who advocate for a single-payer program stood on the sidewalk this morning outside the Renaissance Baltimore Harborplace Hotel, where the president was meeting with House Republicans, and held a large banner reading, “Letting you know: Medicare for all.”

After refusing to move across the street, the two physicians were briefly taken into custody by law enforcement officials, issued a citation and then released.

One of the two was Dr. Margaret Flowers, a pediatrician and congressional fellow for Physicians for a National Health Program, an organization of 17,000 physicians who support a single-payer system, who is trying to deliver an open letter to the president calling on him to meet with her and other Medicare-for-All advocates.

Joining her was Dr. Carol Paris, a practicing psychiatrist and fellow PNHP member from southern Maryland.

In a letter to Obama, Flowers notes how surprised she and others were when single-payer advocates were excluded from the early stages of the discussions on health reform. She and Paris were two of several physicians, nurses and reform advocates who were arrested at Senate Finance Committee hearings last spring for standing up and asking why the Medicare-for-All option was “off the table.”

Flowers writes: “I am asking you to meet with me because the solution is simple. Remove all of the industries who profit off of the American health care catastrophe from the table. Replace them with those who are knowledgeable in designing health systems and who are without ties to the for-profit medical industries. And then allow them to design an improved Medicare-for-All national health system.”

Flowers then itemizes the advantages of adopting such a system, saying that it would cover everyone, save thousands of lives, relieve medical debt, control costs, help the economy, and restore the physician-patient relationship. Obama himself is on record noting only a single-payer plan would provide universal coverage: “The truth is unless you have what’s called a single-payer system in which everyone’s automatically covered, you’re probably not going to reach every single individual.”

En Estados Unidos, 46 millones de personas no tienen seguro de salud porque no pueden pagarlo. 45.000 de ellas mueren anualmente como consecuencia directa de no estar aseguradas, o lo que es lo mismo, una muerte cada 12 minutos que podría evitarse si estas personas tuvieran acceso oportuno a los servicios de salud. El costo promedio del seguro médico para una familia de tres personas es de US $13.000, una suma impagable para un sector cada vez más numeroso de la población, incluyendo a quienes están empleados de tiempo completo. Esta realidad que avergüenza al país económicamente más poderoso del planeta, explica la importancia y la intensidad con que se ha vivido en los últimos meses el debate sobre la reforma al Sistema de Salud, en el cual el presidente Obama se juega su credibilidad y buena parte del apoyo ciudadano, puesto que brindar cobertura universal en salud a bajo costo fue una de sus principales banderas de campaña electoral. Ana Malinow, pediatra que acaba de cumplir su periodo como Presidenta de “Médicos por un Programa Nacional de Salud” (PNHP por sus iniciales en inglés). PNHP es una asociación gremial que agrupa a más de 17.000 médicos y otros profesionales de la salud y que lucha por una reforma de la salud que excluya del sistema la innecesaria intermediación de las compañías privadas de seguros, con lo cual se ahorrarían cada año US $400.000 millones, suficientes para brindar servicios integrales de salud a la totalidad del pueblo estadounidense.

Deslinde: Dra. Malinow, usted es una pediatra dedicada a cuidar sus pacientes. ¿Cuándo se interesó en estudiar el sistema de salud estadounidense y por qué se unió a “Médicos por un Programa Nacional de Salud” (PNHP, por sus siglas en inglés), una organización de la cual usted fue la Presidenta inmediatamente anterior?

Ana Malinow: Me uní a “Médicos por un Programa Nacional de Salud” hace casi una década, pero sólo me hice activa en la organización hasta más tarde, cuando Lauren Oshman, por entonces una estudiante de medicina de tercer año, se acercó a mi oficina. Ella había oído que yo era miembro de PNHP y estaba buscando a un médico que la ayudara a crear una organización para lograr una salud universal en Texas. Tenía un mes para hacerlo. Eso fue en 2002. Yo sabía que se trataba de una tarea casi imposible de coronar en cualquier plazo, más aún en un mes, pero la verdad es que había estado esperando 15 años a que alguien me pidiera esto y, claro está, le dije que sí.

Decidimos que necesitábamos “educar” a la gente sobre el cuidado de la salud y sobre la reforma al sistema de salud. Pero cuando me senté a preparar mi primera presentación, me di cuenta que no sabía nada sobre el sistema de salud de EEUU. Parece extraño, pero después de cuatro años de estudios médicos universitarios, tres años de residencia en pediatría, cinco años de ser una pediatra de cuidados primarios y muchos años de enseñar en una institución académica, no sabía nada sobre el sistema de salud estadounidense. Rápidamente comprendí que primero tendría que autoeducarme. Entonces eso fue exactamente lo que hice. Leí, leí y leí. Y lo que encontré me conmocionó. Desde aquella época he estado hablándole a la gente sobre el sistema de salud.

Por ese entonces, empecé a frustrarme crecientemente con la práctica médica. Estaba gastando más tiempo ayudando a las familias a negociar su atención en un fragmentado sistema de salud que a examinar oídos y escuchar corazones. También oí una historia que creo cambió el curso de mi vida. Una madre me contó sobre su hijo de 15 años de edad que carecía de seguro de salud y quien estaba tratando de ingresar al equipo escolar de baseball. Él estaba trabajando con su madre, limpiando los pupitres y pisos del colegio, con el fin de ganar suficiente dinero para comprarse un par de zapatos de baseball. Su entrenador le dijo que necesitaba un examen médico para poder jugar en el equipo. El muchacho fue donde su madre, quien le explicó que no tenían dinero para pagarle a un doctor por el examen. “No hay ningún problema,” dijo el muchacho, “Yo pagaré con el dinero que he ahorrado para pagarme los zapatos”.

Me indigné. ¿Cómo podía aceptar este tipo de sistema? El país más rico del mundo, ¿y un muchacho tenía que usar su dinero para los zapatos con el propósito de pagar un médico? Fue un momento clave de mi vida. Creo que en ese instante y lugar decidí que no me detendría hasta que el sistema se transformara en uno del cual no me sintiera avergonzada.

Deslinde: ¿Cuál es su diagnóstico sobre el actual sistema de salud de EEUU?

Ana Malinow: Creo que la mayoría de personas están de acuerdo que nuestro sistema de salud está enfermo, es injusto y requiere una reestructuración substancial. De hecho, cuando se les ha encuestado, una gran mayoría de los estadounidenses creen que nuestro sistema de salud necesita un cambio significativo o fundamental. A partir de mi experiencia personal hablándole a cientos y cientos de personas, constato que los americanos están de acuerdo sobre el tipo de sistema de salud que quieren: comprehensivo, costeable, confiable, accesible, universal y justo. Por tanto, el diagnóstico no es difícil; lo que está resultando complicado es la prescripción.

Claro está que creo que la única manera de lograr el tipo de sistema de salud que los americanos desean es a través de un programa nacional de salud de un pagador único. Analizándolo objetivamente, si ello es posible, dejando a un lado la política y la ideología, un sistema de pagador único es el único sistema que podría garantizar un paquete comprehensivo de beneficios costeable, confiable, accesible, universal y equitativo. Todos los demás países desarrollados del mundo lo han descifrado y cayeron en cuenta de ello hace décadas: la salud, para ser universal y costeable, no puede ser una mercancía, comprada y vendida como un aparato de televisión. Tiene que considerarse un servicio público, como los bomberos o la protección policial, como la educación, como las carreteras. Infortunadamente, los estadounidenses han escogido un camino insostenible tratándose de este servicio público. Pero el costo está creciendo, tanto económica como moralmente, y por eso creo que eventualmente prevalecerá el sistema de un pagador único.

Deslinde: ¿Cómo puede compararse el sistema de salud de EEUU con el de otros países desarrollados como Inglaterra, Canadá y Francia?

Ana Malinow: La salud, como todos sabemos, no es solamente el acceso a la atención médica. La salud es más que poder ir al médico y tomarse el medicamento correcto. La salud, como la visualizamos en EEUU, no depende solamente de cuánto dinero un país gasta en el sistema de salud. Por ejemplo, Inglaterra gasta menos de la mitad en términos per cápita que EEUU, y sin embargo tiene una mayor expectativa de vida. Más allá de su constitución genética, la salud depende del barrio en que usted viva, el trabajo que tenga, las disparidades de ingreso entre usted y su jefe, el respaldo social que tenga, las presiones emocionales, financieras, físicas y éticas que enfrenté diariamente, lo que usted coma, tome y respire, y muchos otros aspectos. Por tanto, mirar los resultados de la salud, que constituyen la manera más fácil de comparar los sistemas de salud de los países, para inferir que los sistemas de salud determinan los resultados de salud, probablemente resulta insuficiente cuando no francamente equivocado.

Habiendo aclarado lo anterior, en el año 2000 la Organización Mundial de la Salud clasificó
al sistema de salud de EEUU en el número 37 a nivel mundial y a Francia en el número uno. Adicionalmente al desempeño en salud, midió los niveles en la distribución de la salud, el nivel de respuesta, la justicia en la contribución financiera, los gastos de salud per cápita y los logros generales de metas. En comparación con cualquiera de los países industrializados, EEUU tiene la menor expectativa de vida, la mayor mortalidad infantil y la mayor mortalidad materna. Cuando se comparan el número de médicos y enfermeras, los escáneres MRI y CT, las camas hospitalarias y los pacientes atendidos per cápita, EEUU no clasifica consistentemente entre los mejores países industrializados; de hecho, frecuentemente tiene una clasificación promedio o menor. Al comparar las tendencias en muertes debidas a la atención en salud en 19 países industrializados, el descenso en todos los países es mucho mayor que en EEUU. Si Estados Unidos pudiera reducir la mortalidad debida a la atención en salud al nivel promedio logrado en los tres países con mejores resultados, habría 101.000 muertes menos anuales. Un estudio reciente publicado por los cofundadores de PNHP estima que hay aproximadamente 45.000 muertes anuales en exceso relacionadas con la falta de cubrimiento de salud. Estos son americanos que no habrían muerto si hubiesen tenido un seguro de salud. Cuando se trata de gastar, somos los mejores. Somos los primeros en términos de gastos per cápita en el cuidado de salud.

Como resultado, EEUU gasta anualmente US $2,5 billones (millones de millones) en la salud. En un sistema de salud con ánimo de lucro, hay muy pocos incentivos para mantener los costos bajos. Por tanto, el próximo año gastaremos más y el siguiente todavía más. Mientras que EEUU tenga un sistema de salud con ánimo de lucro, en el cual existen incentivos perversos para seguir gastando más, los costos continuarán aumentando. Infortunadamente, como lo estamos viendo, esto no se traduce en mejorar los resultados de salud. En el futuro, mientras continuemos gastando más dinero en menos población, nuestros resultados generales de salud probablemente empeorarán.

Deslinde: ¿En qué momento el gobierno estadounidense le entregó el sistema de salud al capital financiero, las empresas aseguradoras y las corporaciones del negocio de la salud?

Ana Malinow: Sería difícil precisar un evento o fecha, pero creo que el nacimiento y la perniciosa persistencia de los seguros de salud con ánimo de lucro en EEUU figuran entre los múltiples determinantes que propenden sinergéticamente hacia el espiral de muertes que actualmente estamos experimentando en el sistema de salud.

Hay tres “actores malos” en esta historia, todos motivados por la avaricia: el poder no cuestionado de la Asociación Médica Americana, el ascenso y caída de la Blue Cross/Blue Shield y el surgimiento de los seguros de salud con ánimo de lucro.

Antes de la década de 1920, EEUU carecía de un sistema organizado de hospitales, médicos o seguros de salud y la mayoría de gente pagaba directamente por la atención de salud. En 1930, la Asociación Americana de Hospitales estableció la Blue Cross y en 1939 los médicos en California establecieron la Blue Shield, lo cual resultó en que muchos estadounidenses gozaran de un seguro de salud, pero más importante que lo anterior, en que hubo una fuente constante de ingresos para los hospitales y los médicos a pesar de la depresión nacional. Otorgarle a los hospitales y médicos el control del aseguramiento en salud garantizó que el reembolso fuera generoso y que la contención de costos se relegara.

La Blue Cross fue creada como una empresa sin ánimo de lucro, regulada por el gobierno y con la obligación de ofrecer seguros de salud a nivel comunitario, garantizando a todas las personas iguales servicios, sin importar sus condiciones de edad, médicas o de ingreso. Durante la Segunda Guerra Mundial, debido al congelamiento de salarios y precios, los empleadores empezaron a atraer trabajadores ofreciéndoles beneficios de salud en vez de aumentar sus salarios. De esta manera empezó el aumento de un siglo en los seguros de salud patrocinados por los empleadores, quienes continúan ofreciendo beneficios de salud a sus trabajadores como una especie de salario diferido.

En la década de 1940, el éxito de los seguros de salud patrocinados por los empleadores atrajo a las empresas comerciales de seguros al negocio de los seguros de salud, empresas que tenían como negocio obtener mayores ganancias que los “Blues”. En vez de una tasación basada en la comunidad, las empresas de seguros comerciales usaron una tasación basada en la experiencia, determinando las primas de seguros según la experiencia, edad y condiciones de salud de los trabajadores.

Eventualmente, para sobrevivir a la competencia no regulada, la Blue Cross no tuvo otra opción que abandonar la tasación basada en la comunidad por la tasación basada en la experiencia, dejando sin seguro a los enfermos, personas de la tercera edad y pobres que no podían pagar las costosas primas basadas en la experiencia.

Esto se convirtió en un legado de “Salud para los Saludables”, que constituye buena parte del actual sistema de salud estadounidense. Los seguros de salud fueron creados (es cierto, para garantizar clientes, pero también) como un esfuerzo de la sociedad por solucionar el problema de un sistema de salud incosteable en términos del sistema de pago en efectivo. Un sistema de salud más costeable condujo a los abusos en su utilización, primero porque los bien asegurados no tenían incentivos para no usar el sistema de salud, y segundo porque existían pocos incentivos por parte de los médicos y hospitales para no utilizar el sistema de salud, teniendo en cuenta que a ellos les pagaban sus propias facturas. Los costos y las ganancias aumentaron. Esto funcionó para una sociedad con una tasación basada en la comunidad, en la cual existían subsidios cruzados, pero para mediados del siglo XX los seguros de salud recurrieron a una tasación basada en la experiencia, manteniendo a los saludables en un sistema que les brindaba salud y excluyendo a los enfermos, pobres y personas de la tercera edad. Todo el mundo circuló por el sistema de salud: quienes tuvieron suerte quedaron cubiertos, mientras que los infortunados quedaron excluidos. Para finales de la década de 1950, únicamente un 50% de las personas de la tercera edad tenían seguro de salud. ¿Qué se podía hacer con los pobres y las personas enfermas de la tercera edad que no podían pagar un seguro de salud? ¡El gobierno al rescate! En 1965, con la aprobación de Medicare para las personas de la tercera edad y de Medicaid para los pobres, el gobierno asumió la responsabilidad respecto de los elementos más costosos de atención médica, dejándoles las ganancias a las empresas de seguros privadas.

La razón por la cual incluyo a la Asociación Médica Americana (AMA) en este grupo de los “actores malos” es porque ella ha detentado un gran poder, especialmente durante el último siglo. En vez de usar su poder en pro de una reforma que favoreciera a los pacientes, consistente y rotundamente persiguió sus propios intereses contra los de un sistema de salud universal. Utilizó su no insignificante poder económico y de cabildeo para oponerse a cualquier reforma del sistema de salud desde 1916. Se opuso fervientemente a la aprobación de Medicare y hasta hace muy poco también a un sistema de salud universal. Afortunadamente, los líderes actuales parecen haberse iluminado y están trabajando con el presidente Obama para reformar el sistema de salud. Creo que la explicación de este cambio reciente es que la AMA se ha dado cuenta de que aunque puede tener dinero no tiene el respaldo de los médicos en EEUU. Menos del 30% de los médicos pertenecen a la AMA (algunos cálculos hablan de un porcentaje tan bajo como el 19%). Un estudio reciente publicado en los Anales de M
edicina Interna, muestra que el 59% de los médicos estadounidenses escogidos al azar favorecen una legislación que establezca un programa nacional de salud. Esto significa un aumento del 10% en el apoyo a un sistema de pagador único en sólo 5 años.

Deslinde: ¿Cuál ha sido el papel de las empresas farmacéuticas en la actual crisis de salud en EEUU?

Ana Malinow: El papel de las empresas farmacéuticas siempre ha sido proteger sus intereses. El surgimiento de las empresas farmacéuticas empezó en 1980, cuando Ronald Reagan fue elegido presidente. Fue un año seminal, porque si usted mira el crecimiento de los gastos nacionales en productos farmacéuticos, comprobará que hubo un fuerte aumento sin precedentes empezando 1980, cuando los gastos totalizaron US $8 mil millones anuales. Para 1985, los gastos por drogas crecieron a US $22 mil millones y para 1990 a US $40 mil millones. El único otro momento en que observamos un crecimiento tan exponencial en los gastos nacionales farmacéuticos es después de 2000, con la elección de George W. Bush. Para 2004, EEUU estaba gastando US $188 mil millones anuales en fármacos.

Estimulando los crecientes gastos en medicamentos de EEUU estuvieron cuatro leyes, empezando con el Acto BayhDole en 1980, el cual aceleró el proceso de comercialización de la investigación universitaria financiada federalmente; el Acto Hatch-­–Waxman en 1984, que promovió ostensiblemente las drogas genéricas pero que sobre todo otorgó periodos más largos de exclusividad en el mercadeo de los medicamentos a las empresas farmacéuticas; el establecimiento de la propaganda directa al consumidor en 1997, la cual está prohibida en la mayoría de países, medida que significó agregar más de US $3 mil millones anuales a los cofres de la industria farmacéutica; y el Acto de Modernización de Medicare de 2006, que permitió la prescripción de medicamentos en Medicare (también conocido como Parte D), pero debido a que esta norma prohíbe que Medicare negocie directamente con las empresas farmacéuticas, conlleva vaciar el dinero de los contribuyentes directamente en los cofres siempre en aumento de la industria farmacéutica. En los primeros seis meses después de enero 1o de 2006, cuando entró en vigencia el programa de medicamentos de Medicare, las ganancias de la industria farmacéutica aumentaron más de US $8 mil millones.

Estas cuatro leyes hicieron más para aumentar las ganancias de las empresas de medicamentos en detrimento del público americano que cualquier otra legislación en la historia. Con la aprobación de cada nueva ley, las ganancias de la industria aumentaron, los esfuerzos de cabildeo se hicieron mayores y se aprobaron más leyes a favor de la industria. El resultado ha sido un crecimiento exponencial de los gastos de salud nacionales en productos farmacéuticos.

Es interesante notar que uno de los extraños acompañantes del actual debate sobre la reforma del sistema de salud estadounidense ha sido Big Pharma, representante oficial de la industria farmacéutica, la cual está gastando US $150 millones en una campaña de propaganda a favor de la reforma de salud de Obama. En un acuerdo detrás de telones, Big Pharma está poniéndose de acuerdo para contribuir US $80 mil millones durante 10 años para costear el programa de salud, siempre y cuando la Casa Blanca acepte respaldar varios negocios de la industria farmacéutica, incluyendo oponerse a la reimportación de medicamentos desde Canadá y apoyar el status-­–quo frente a Medicare Parte D. Pero US $80 mil millones durante 10 años son poco comparados con las ganancias de Big Pharma. Un análisis realizado por CBO sobre los precios de los medicamentos de prescripción en Medicare Parte D, muestra que los contribuyentes podrían ahorrarse US $370 mil millones durante diez años si se permite que Medicare negocie precios como lo hace la Administración de Veteranos, entidad prestadora de servicios de salud a los veteranos de las fuerzas militares de Estados Unidos. La industria entrega US $80 mil millones pero retiene US $290 mil millones. A diferencia del gobierno estadounidense, Big Pharma sabe lo que está haciendo.

Deslinde: ¿Puede resumir la posición del presidente Obama en relación con la reforma de salud actualmente en discusión?

Ana Malinow: La posición del presidente Obama frente a la reforma de salud puede sintetizarse en cuatro puntos claves:

Uno, incluirá un mandato individual en relación con los subsidios. Todas las personas en EEUU por encima de cierto nivel federal de pobreza estarán obligadas a tener un seguro de salud privado, o tendrán que pagar una multa de miles de dólares cada año. Infortunadamente, las matemáticas no concuerdan. Con un ingreso de US $49.000 de los hogares promedios estadounidenses y un costo promedio de US $13.375 de las primas de salud familiares, la familia típica americana tendrá que gastar hasta el 27% de su ingreso anual en seguros de salud. Eso para 2009. Dado que el seguro de salud privado ha aumentado 131% en diez años, más de tres veces que los salarios y cuatro veces que la inflación, las primas de seguros de salud probablemente acapararán una mayor parte de los ingresos de los trabajadores americanos en el futuro. Dos, el mandato de los empleadores. La mayoría de empleadores necesitarán ofrecer algún tipo de seguro de salud a sus trabajadores o enfrentarán multas. Detalles como qué proporción pagarán los empleadores, qué tan comprehensivo será el plan y la modalidad de deducción atada al plan, deberán acordarse en algún momento. Ciertamente, algunos empleadores también recibirán subsidios tributarios para que ofrezcan seguros de salud a sus empleados.

Tres, la expansión de Medicaid y del Plan de Seguros de Salud para Niños (CHIP en su sigla en inglés). En promedio, los Estados financian aproximadamente el 40% de todos los gastos de Medicaid y el 30% de todos los gastos del CHIP. Estados que tradicionalmente no han sido muy generosos con sus programas públicos, como Texas y otros Estados sureños, encontrarán enormemente costoso expandir Medicaid y el CHIP. Entre mayor sea el nivel de pobreza en el Estado, mayor será la cantidad de personas elegibles para Medicaid y el CHIP. Paradójicamente, se espera que los Estados pobres, con bajos ingresos, paguen una parte significativa de esta reforma.

Cuatro, un “intercambio” nacional, que ofrecerá algún tipo de seguro de salud privado regulado, y posiblemente algún tipo de opción pública de seguro a aquellos individuos que no estén cubiertos por el seguro de salud patrocinado por el empleador. Estos seguros no podrán discriminar por razones de condiciones médicas preexistentes y tendrán prohibido incurrir en políticas de rescisión, esto es, políticas de cancelación de servicios debidas a costos excesivos de salud en que incurra la empresa aseguradora. No es claro cuánto costarán estas políticas y qué tipo de controles de costo existirán.

Supongo que esta modalidad de enfoque tibio sobre la reforma del sistema de salud podría funcionar si fuera sostenible, que no lo es. Las medidas de control de costos propuestas por el presidente Obama (registros médicos electrónicos, ahorros en Medicare y Medicaid, etc.) nunca estarán cerca de financiar este sistema abstruso, que se ahogará debido a su propia ineficiencia y desperdicio extremo.

El presidente Obama está modelando su reforma de salud según el esfuerzo de reforma fallida de Massachusetts en 2007. El único ganador allí ha sido Blue Cross/Blue Shield, el cual se está embolsillando US $1 millones mensuales. La reforma del Presidente no es más que otro costoso rescate corporativo, esta vez en favor de la industria de seguros de salud.

eslinde: ¿Cuál es su evaluación del discurso sobre la reforma a la salud, pronunciado ante el Congreso por el presidente Obama?

Ana Malinow: Estoy de acuerdo con el Presidente de que el cuidado de la salud es un asunto moral, pero no concuerdo con él en cómo reformar el sistema de salud. No necesitamos botar más dinero bueno después de haber desperdiciado dinero malo. Ya sobrepasamos a prácticamente todos los otros países industrializados casi por el doble y constatamos que gastar dinero no se traduce en mejorar los resultados de salud. La industria privada de seguros de salud necesita hacer algo más que vender seguros de salud. No añade nada a nuestra salud. Es innecesaria y constituye un impedimento para una verdadera reforma. Desearía que el Presidente hubiera dicho en su discurso al Congreso y hubiera actuado en consecuencia, que iba a aprobar una legislación para expandir Medicare a favor de todos. Eso es realmente todo lo que se necesitaría. Una ley de un solo artículo: El Congreso expande Medicare a todos los estadounidenses.

Deslinde: Si Obama no está proponiendo una reforma comprehensiva e integral que afecte los intereses de las grandes empresas de seguros y farmacéuticas, ¿cómo puede explicar la campaña radical emprendida por organizaciones del ala derecha contra la propuesta del Presidente?

Ana Malinow: Creo que la campaña radical lanzada por las organizaciones del ala derecha contra la propuesta de salud del presidente Obama tiene poco que ver con su propuesta de salud. Hay dos movimientos: el intenso cabildeo en Washington adelantado por la industria de seguros de salud, la cual está gastando diariamente US $1 millones cabildeando en el Congreso, y la campaña del ala radical contra el propio presidente Obama. No debemos confundir las dos.

Usted nota que Big Pharma no está gastando millones para desprestigiar al Presidente, de hecho, es todo lo contrario: está contribuyendo millones para respaldar las iniciativas presidenciales. La Asociación de Planes de Seguros de Salud, el grupo de las industrias de seguros, ha estado en todas las audiciones y reuniones que el Congreso ha efectuado sobre el sistema de salud. Si las propuestas del Presidente pasan, las industrias farmacéuticas y de seguros de salud estarán más que contentas. Sería su rescate. Están gastando millones cabildeando a favor del Presidente, no en su contra.

La derecha radical ha asumido el tema de la salud como su grito de guerra; eso era lo que estaba ocurriendo en agosto. ¿Quién está financiando a los “Tea Baggers”, los “Birthers” y a otros extremistas de derecha? Personas como Dick Armey, anterior líder de la mayoría de la Cámara de Representantes, quien ahora dirige FreedomWorks, una organización que promueve manifestaciones racistas en reuniones a lo largo de todo el país. Corporaciones como Koch Industries, que hace generosas donaciones tanto a Americans for Prosperity como al grupo de Dick Armey. Estas campañas, basadas en la decepción, el racismo, el sexismo y el odio, están orquestando las protestas contra Obama. Que estos individuos protesten contra la posición de salud del Presidente es consistente con su ideología, la cual incluye oponerse a cualquier cosa defendida por Obama. Su mensaje es irracional, como el gerente que grita, “¡Mantenga las manos del gobierno lejos de mi Medicare!”.

Deslinde: ¿Cuáles son los principales componentes del Seguro Nacional de Salud de pagador único, propuesto por PNHP y otras organizaciones?

Ana Malinow: Los principales componentes del sistema nacional de salud de pagador único, o “Medicare para Todos”, están materializados en la HR 676, legislación introducida por los Representantes Conyers y Kucinich. Ella resultaría en  
un sistema de salud universal, comprehensivo, confiable, accesible, costeable y equitativo.

Significa que todos estén incluidos, que nadie quede por fuera. Es portátil, de empleo a empleo, de empleo a desempleo, de Estado a Estado. Tiene beneficios uniformes y comprehensivos; incluye todos los servicios internos y externos a los pacientes: dentales, de visión, de salud mental, medicamentos, equipos médicos, servicios de abuso de drogas y cuidados de larga duración. Promueve el cuidado primario y apoya la prevención. Permite escoger médicos y hospitales. Ana Malinow: Hay dos propuestas de pagador único en el Congreso: la S703, que es la propuesta de Sanders en el Senado, y la HR 676, que es la propuesta de Kucinich-­–Conyers en la Cámara. A la HR 676 la apoyan 86 copatrocinadores. Existen dos modificaciones muy importantes, una modificación de substitución presentada por el representante de Nueva York, Anthony Weiner, para reemplazar el lenguaje de la HR 3200 de la Cámara con el lenguaje de la HR 676 que habla de un pagador único, modificación que será debatida y votada en la Cámara en otoño. También está la modificación de Kucinich en la HR 3200, la cual le permitiría a los Estados la opción de promover programas de pagador único. Será una votación histórica, dado que el asunto del pagador único nunca había sido debatido en el Congreso. Pero el apoyo en el Congreso sólo será tan fuerte como lo hagan los constituyentes. Estamos tremendamente superados en número por los cabildantes; seis de ellos por cada miembro de la Cámara y del Senado—3.300 cabildantes que ejercen influencia sobre el sistema de salud, y tres nuevos cabildantes uniéndose a la lucha diariamente. La oposición que enfrentamos es tremenda y no debe subestimarse.

Deslinde: ¿Qué acciones están emprendiendo las organizaciones que proponen el sistema de pagador único para presionar una reforma comprehensiva del sistema de salud?

Ana Malinow: Me enorgullezco profundamente de lo que los defensores del sistema del pagador único han logrado. Recuerde, esta lucha se viene
librando desde 1916 y la oposición sólo se ha hecho más fuerte y rica. Hemos visto que, como ocurre con la industria farmacéutica, entre más ricos se hacen, se tornan más fuertes. Pero a pesar de todos los cabildantes y millones, los defensores del sistema de pagador único han triunfado en lograr que dos modificaciones y propuestas lleguen hasta ambas cámaras parlamentarias por primera vez en la historia. Hemos logrado que políticos como el senador Max Baucus y la representante Nancy Pelosi reconozcan que el sistema de pagador único es popular. Hasta el presidente Obama dijo, “Si pudiéramos empezar de cero, optaría por el pagador único”. Eso no ocurrió por que los cabildantes se lo hayan pedido. Más de 500 sindicatos locales han respaldado el sistema de pagador único, como lo han hecho los gobiernos de ciudades importantes como Philadelphia y Boston, San Francisco y Detroit, el Consejo Nacional de Alcaldes e importantes congregaciones de fe.

Los activistas del sistema de pagador único están realizando actos de desobediencia civil. Médicos y enfermeras fueron arrestados durante las audiencias del Comité de Finanzas, encabezado por el senador Baucus. Even before Senator-elect Scott Brown’s stunning victory in the Massachusetts special election, there was no shortage of advice on simpler approaches to health reform that the president and his Congressional allies could have used in place of the bills now before Congress.

Although the current reform bills undoubtedly are burdened with many tangential items, one can easily underestimate how quickly any kind of health reform will become complex.

(He quotes Peggy Noonan who advocates for “small, discrete steps” such as “a simple bill that mandated insurance companies offer coverage without respect to previous medical conditions.”)

In the eyes of people unfamiliar with economics, the step she (like others before her) proposes may seem small.

She seems completely unaware that, to be implemented, that step has to be accompanied by (1) a mandate to be insured or, at the least, very powerful financial incentives to be insured. And if government imposes such a mandate on citizens, it must be ready (2) to subsidize low-income families in the acquisition of the mandated insurance. Already we have a bill requiring many pages.

(He quotes from Sen. Tom Coburn’s summary of his bill calling for guaranteed coverage, and affordable premiums through risk adjustment.)

The European systems Mr. Coburn has in mind are the Swiss and Dutch health systems, because we are referred to a paper on these systems.

It should be noted that the Dutch and especially the Swiss systems are subject to heavy government regulation — far heavier than is foreseen in the Senate health reform bill passed in late December.

For example, health insurance is mandatory in both countries. Both countries prescribe purely community-rated insurance premiums which, unlike the Senate bill, cannot take the age of the insured into account. And both countries extend sizable public subsidies toward the purchase of private health insurance.

In short, as Senator Coburn’s bill illustrates, “simply” to pass a bill that imposes community rating on health insurers, as Ms. Noonan suggests, is anything but simple. It opens a veritable Pandora’s box of additional, required legislation which, once fully fleshed out, would pretty much constitute the core of the insurance-reform bill currently in the Senate bill.

Once Congress decided to build on our existing, fragmented, dysfunctional system of financing health care, it became necessary to create the complex legislation that was generated in both the House and the Senate, precisely because of the reasons Prof. Reinhardt describes.

Congress and the administration began with the most expensive system in existence and then attempted to apply social insurance principles to our hybrid model, allegedly with the goal of covering everyone with a system with sustainable spending. The health policy literature is replete with simulations and real-life studies demonstrating that this is by far the most expensive model of reform, and it falls short on universality, equity, and efficiency.

The least expensive and most effective models are a government-owned health service (socialized medicine, not America’s first choice), or a bona fide social insurance program used to finance the largely private health care delivery system. Although there is no perfect system, a government social insurance program (like Medicare) does have an advantage over social insurance programs using a market of private plans in that administrative costs are lower and mechanisms to moderate spending increases are more effective. Also, the Swiss and Dutch private insurance-based programs still leave uninsured about 1.5 percent of their populations.

President Obama this week told the nation that he was eager to see a better approach to cover everyone and bring down costs, though in his prior comments he has revealed that he already knows how to do that. And what is that?

During this break in the negotiations on reform, we really should take a serious look at an improved Medicare program that would cover everyone. It is the least expensive model of reform. It has the greatest administrative efficiency. It would truly cover everyone automatically. And even for those who do not support social solidarity, it would finally control our intolerable growth in health care spending through the proven mechanism of a single payer monopsony.

To the Editor:

President Obama’s State of the Union address had a high point when he pledged that anyone with a “better approach that will bring down premiums, bring down the deficit, cover the uninsured, strengthen Medicare for seniors, and stop insurance company abuses, let me know.”

Thank you, Mr. President. The answer is the reform supported by 65 percent of the public and even 59 percent of physicians. It’s remarkably simple, and the nation has already had 44 years of successful experience with it in financing health care for our elderly and the totally disabled.

It is, of course, Medicare-for-all, single-payer, not-for-profit national health insurance. Its superiority lies in excluding profit-seeking insurance companies and Big Pharma from controlling and undermining our health system. This is your answer, Mr. President.

Rising to President Obama’s challenge to others in his State of the Union address that they come up with a better approach to health care reform than his own, physicians who advocate for a single-payer program stepped forward this morning to again make the case for their alternative, which they say has solid public support.

Among them is Dr. Margaret Flowers, a pediatrician and congressional fellow for Physicians for a National Health Program, an organization of 17,000 physicians who support a single-payer system, who is traveling to the White House today to deliver an open letter to the president calling on him to meet with her and other Medicare-for-All advocates.

Also speaking out today are Drs. Steffie Woolhandler and David Himmelstein, co-founders of PNHP, primary care physicians in Cambridge, Mass., and professors at Harvard Medical School, who provided commentary in a blog in today’s New York Times.

In her letter to Obama, Flowers notes how surprised she and others were when single-payer advocates were excluded from the early stages of the discussions on health reform. Flowers was one of several physicians, nurses and reform advocates who were arrested at Senate Finance Committee hearings last spring for standing up and asking in a dignified way why the Medicare-for-All option was “off the table.”

Flowers writes: “I am asking you to meet with me because the solution is simple. Remove all of the industries who profit off of the American health care catastrophe from the table. Replace them with those who are knowledgeable in designing health systems and who are without ties to the for-profit medical industries. And then allow them to design an improved Medicare-for-All national health system.”

Flowers then itemizes the advantages of adopting such a system, saying that it would cover everyone, save thousands of lives, relieve medical debt, control costs, help the economy, and restore the physician-patient relationship. Obama himself is on record noting only a single-payer plan would provide universal coverage: “The truth is unless you have what’s called a single-payer system in which everyone’s automatically covered, you’re probably not going to reach every single individual.”

Replace them with those who are knowledgeable in designing health systems and who are without ties to the for-profit medical industries. And then allow them to design an improved Medicare-for-All national health system.”

Flowers then itemizes the advantages of adopting such a system, saying that it would cover everyone, save thousands of lives, relieve medical debt, control costs, help the economy, and restore the physician-patient relationship. Obama himself is on record noting only a single-payer plan would provide universal coverage: “The truth is unless you have what’s called a single-payer system in which everyone’s automatically covered, you’re probably not going to reach every single individual.”

Dear President Obama,

I was overjoyed to hear you say in your State of the Union address last night:

“But if anyone from either party has a better approach that will bring down premiums, bring down the deficit, cover the uninsured, strengthen Medicare for seniors, and stop insurance company abuses, let me know.”

My colleagues, fellow health advocates and I have been trying to meet with you for over a year now because we have an approach which will meet all of your goals and more.

I am a pediatrician who, like many of my primary care colleagues, left practice because it is nearly impossible to deliver high quality health care in this environment. I have been volunteering for Physicians for a National Health Program ever since. For over a year now, I have been working with the Leadership Conference for Guaranteed Health Care/ National Single Payer Alliance. This alliance represents over 20 million people nationwide from doctors to nurses to labor, faith and community groups who advocate on behalf of the majority of Americans, including doctors, who favor a national Medicare-for-All health system.

I felt very optimistic when Congress took up health care reform last January because I remember when you spoke to the Illinois AFL-CIO in June, 2003 and said:

“I happen to be a proponent of a single-payer universal health care program.” (applause) “I see no reason why the United States of America, the wealthiest country in the history of the world, spending 14 percent of its Gross National Product on health care cannot provide basic health insurance to everybody. And that’s what Jim is talking about when he says everybody in, nobody out. A single-payer health care plan, a universal health care plan. And that’s what I’d like to see. But as all of you know, we may not get there immediately. Because first we have to take back the White House, we have to take back the Senate, and we have to take back the House.”

And that is why I was so surprised when the voices of those who support a national single-payer plan/Medicare for All were excluded in place of the voices of the very health insurance and pharmaceutical industries which profit off the current health care situation.

There was an opportunity this past year to create universal and financially sustainable health care reform rather than expensive health insurance reform. As you well know, the United States spends the most per capita on health care in the world yet leaves millions of people out and receives poor return on those health care dollars in terms of health outcomes and efficiency. This poor value for our health care dollar is due to the waste of having so many insurance companies. At least a third of our health care dollars go towards activities that have nothing to do with health care such as marketing, administration and high executive salaries and bonuses. This represents over $400 billion per year which could be used to pay for health care for all of those Americans who are suffering and dying from preventable causes.

The good news is that it doesn’t have to be this way. You said that you wanted to “keep what works” and that would be Medicare. Medicare is an American legacy of which we can feel proud. It has guaranteed health security to all who have it. Medicare has lifted senior citizens out of poverty. Health disparities, which are rising in this nation, begin to disappear as soon as patients reach 65 years of age. And patients and doctors prefer Medicare to private insurance. Why, our Medicare has even been used as a model by other nations which have developed and implemented universal health systems.

Mr. President, we wanted to meet with you because we have the solution to health care reform. The United States has enough money already and we have the resources, including esteemed experts in public health, health policy and health financing. Our very own Dr. William Hsiao at Harvard has designed health systems in five other countries.

I am asking you to meet with me because the solution is simple. Remove all of the industries who profit off of the American health care catastrophe from the table. Replace them with those who are knowledgeable in designing health systems and who are without ties to the for-profit medical industries. And then allow them to design an improved Medicare-for-All national health system. We can implement it within a year of designing such a system.

What are the benefits of doing this?

* It will save tens of thousands (perhaps hundreds of thousands) of American lives each year, not to mention the prevention of unnecessary suffering.

* It will relieve families of medical debt, which is the number one cause of bankruptcy and foreclosure despite the fact that most of those who experienced bankruptcy had health insurance.

* It will relieve businesses of the growing burden of skyrocketing health insurance premiums so that they can invest in innovation, hiring, increased wages and other benefits and so they can compete in the global market.

* It will control health care costs in a rational way through global budgeting and negotiation for fair prices for pharmaceuticals and services.

* It will allow patients the freedom to choose wherever they want to go for health care and will allow patients and their caregivers to determine which care is best without denials by insurance administrators.

* It will restore the physician-patient relationship and bring satisfaction back to the practice of medicine so that more doctors will stay in or return to practice.

* It will allow our people in our nation to be healthy and productive and able to support themselves and their families.

* It will create a legacy for your administration that may someday elevate you to the same hero status as Tommy Douglas has in Canada.

Mr. President, there are more benefits, but I believe you get the point. I look forward to meeting with you and am so pleased that you are open to our ideas. The Medicare-for-All campaign is growing rapidly and is ready to support you as we move forward on health care reform that will provide America with one of the best health systems in the world. And that is something of which all Americans can be proud.

Did Obama Move Health Care Forward?

Going Down the Same Old Tunnel

Having surrendered in advance to the private insurers and drug companies who profit from our dysfunctional health financing system, President Obama and the Democrats who lead Congress couldn’t rally the American people to support their woeful plan against Republican attacks.

In the end, the Democrats’ back room deals produced a scheme reminiscent of the ones crafted by Mitt Romney (in Massachusetts, 2006) and Richard Nixon (Washington, 1972) – but even less effective than those.

A simple single payer program could save $400 billion annually on insurance-related bureaucracy and profits, and tens of billions more by driving down drug prices. That’s more than enough to cover the uninsured and to upgrade coverage for insured Americans, without increasing health spending.

But the Democrats’ efforts to appease insurers and pay off the pharmaceutical industry made covering the uninsured unaffordable. Once they’d rejected single payer (and its savings on useless bureaucracy), the Democrats could only offer unpalatable funding options; stick consumers with ever-higher health costs, raise taxes, or drain money from care. In the end, the Democrats chose all three.

Their individual mandate would force millions to pay private insurers’ outrageous premiums for coverage so skimpy that a major illness would still lead to bankruptcy. (And if you currently have coverage you don’t like, they’d force you to keep it). They added a steep excise tax on workers lucky enough to still have good coverage. And their massive Medicare cuts included a plan to take billions from already-strapped public hospitals (and other safety net providers) and give it to private insurers. In sum, the Democrats asked middle class Americans to pay more, and get little in return.

The president can regain his footing by reconnecting with the hopes of Americans who elected him and rejecting the sordid corporate compromises that signify Washington politics as usual. A single-payer, Medicare-for-All reform would lower costs, cover the uninsured, and upgrade coverage for most insured Americans. Leading a crusade for such a plan, he’d mobilize vast popular support; enough to overwhelm Republican obstructionism. But instead, he seems intent on looking for light at the end of the same old tunnel.

During his campaign, President Obama declaimed that he’d back single payer – if we were starting from scratch. One thing’s clear from the recent Massachusetts election upset: it’s time to start from scratch.

Healthways SilverSneakers is a membership fitness program for seniors provided as a free benefit by many Medicare Advantage programs throughout the nation. It is not a benefit that is allowed in the traditional Medicare program.

We certainly applaud seniors who participate in programs designed to maintain physical fitness and general wellness. But there are two features of this program that should outrage taxpayers.

1) These programs are funded by government overpayments made to the private Medicare Advantage plans. Though some of the overpayments are used for additional benefits, much is wasted in excessive administrative costs and profits. Not only should taxpayers object to the waste, they should also object to a program that provides extra benefits limited to private plan enrollees but not available to those in the traditional Medicare program. If the extra benefits are appropriate, then all Medicare beneficiaries should receive them. If they are not appropriate, then no taxpayer funds should be used for these benefits. In either case, the overpayments directed to the private plans should either be redistributed amongst all Medicare beneficiaries or retained in our Medicare reserves. Regardless, the private Medicare Advantage plans should be eliminated as the wasteful intermediaries that they’ve proven to be.

2) Which of our seniors would find the SilverSneakers program to be an attractive benefit? Obviously only the healthiest without infirmities that would prevent them from participating in the exercise programs. SilverSneakers is being selectively marketed to the healthiest, lowest-cost seniors, even though their private plans are being paid much more than we spend taking care of patients in the traditional Medicare program which includes a disproportionate share of chronically ill patients. The taxpayers take care of the sick, while they doubly overpay the private plans to take care of the lower-cost healthier individuals.

The Republicans are feigning outrage by protesting that the proposal before Congress would “cut Medicare spending” by partially reducing some of the overpayments to the Medicare Advantage plans. Well, the Democrats actually are wrong. In a brave show of timidity, they suggest tweaking the Medicare Advantage overpayments, when they should show the temerity to throw those crooks out and use the funds to improve benefits in the traditional Medicare program.

While they’re at it, they should throw out the crooks running the rest of the private plans, and establish an improved Medicare program for all of us.

What about the seniors who would lose their SilverSneakers membership? They can buy a program directly from Curves or Bally Total Fitness, thereby eliminating both the SilverSneakers and the Medicare Advantage middlemen. Or better yet, they can organize their own fitness groups and continue to exercise – for free!

Message from Massachusetts

Well, that was a game-changer! But don’t misinterpret it (and don’t blame Martha Coakley’s lackluster campaign). Scott Brown’s victory was not about the principles of either party, nor was it about the size of government, nor even about health reform, except indirectly. It was about disillusionment and anger with government.

When Obama became president, there was a euphoric sense that we really would see “change you can believe in.” Yes, there were unrealistic expectations — in the words of John Stewart’s “senior black correspondent,” a belief in “miracle Negroes,” even though they’re “just guys.” But for the most part, people did not expect overnight solutions. What they counted on was that Obama would set to work keeping his campaign promises, and do so in a forthright and transparent way.

So what happened? Right out of the chute, he signaled that there might be very little change. After appointing the usual Goldman Sachs crowd as advisors, he showered money on Wall Street unconditionally, while doing relatively little for people who were losing their houses and jobs. Nor did he do much for the states, whose budget woes people feel directly and are a big part of what happened on Tuesday. He did some good things, of course, but they were mostly small and on the margins.

And the campaign promises? We still have both wars raging, with about the same number of troops, just re-distributed from Iraq to Afghanistan. We still have Guantanamo, we still have indefinite imprisonment without charges, and we almost certainly still have torture. As for transparency, there’s still not much of it. And the federal deficit continues to climb, despite the lack of palpable relief for Main Street. Not everyone knows all of this, or cares, but some people know each of these things, and together, they add up to a lot of disillusioned people.

As for health care, not many of the angry voters understand all the provisions of the Senate bill (who does?). But they do know that Obama has “pissed away” a year on it, as Theda Skocpol says, much of it devoted to obsequious courting of the likes of Ben Nelson and Joe Lieberman and Olympia Snow. That raised real questions about his abilities as a leader. And most people under 65 know that the reform would require them to buy commercial insurance, a prospect nearly everyone dislikes. Finally, they almost certainly have some inkling that the bill was written by lobbyists. After all, last spring the White House hosted a highly publicized photo-op and jamboree with the health industries, at which they pledged to work together.

Liberals are wrong to think that opposition to health reform is a rejection of big government. If health reform consisted of extending Medicare to everyone, people would be delighted. There are millions of 64-year-olds out there who can hardly wait to be 65. (And if the government created lots more jobs, they would be delighted by that, too.)

It’s not the size of government; it’s how well it works and on whose behalf. Many people no longer trust Obama to work in their interests, or even to tell the truth. They vote for Republicans mainly because they’re the only other game in town. But I don’t think that means they subscribe to Republican principles. In Massachusetts, Republicans constitute about 11 percent of voters, and I doubt whether that number will increase by much. People are rejecting both parties in droves and becoming independents, because they no longer believe government works for them, no matter which party is in charge. The same dynamic that swept Obama into office might well sweep him out.

It’s also mistake to lump all the protesters together as Sarah Palin yahoos. There are many of those, of course, but in addition there are many people who, even if they’re confused enough to vote Republican, have legitimate concerns, not all of them the same — including Ron Paul libertarians who want out of Iraq and Afghanistan, deficit hawks who worry about saddling their kids with more debt, and ordinary people who are just worried or desperate and don’t see any relief coming from government.

If they don’t draw the wrong lessons from the Scott Brown debacle, the Democrats might still recover. The problem is not that Obama tried to do too much, but that he compromised away his principles.
